Centra PACE Lynchburg is a national program of comprehensive care for the elderly which promotes independence and the highest levels of functioning while allowing choice and dignity for members and their families.

Criteria for eligibility: age 55 or older, qualify for nursing facility level of care in the state of Virginia, able to live safely in the community with the support of PACE, live within a 30 minute radius from downtown Lynchburg. Centra PACE Lynchburg mission is to enhance life for older adults with chronic needs by providing compassionate, quality care and services. Participants are empowered to live in their homes and in the community for as long as it is medically and socially safe.

Comprehensive services are delivered by a team made up of a primary care physician, RN’s, social worker, health aides, transportation personnel, rehabilitation therapists, dietitians and recreation activity personnel.

Funded through a unique arrangement with Medicare and Medicaid; some long term care insurance accepted, private pay options available.

  • Medical care and transportation to and from specialists
  • Adult day care, wheelchair-accessible transportation
  • Hospital care (as determined by the PACE care team), 24-hour on-call physician services
  • Prescription medicines, durable medical equipment
  • Vision, hearing, and dental care
  • Rehabilitation and therapy services
  • Outpatient medical services such as lab work, x-rays
  • Home care and nutritional services
  • Meals
  • Personal emergency response
