We’re merging the expertise of Central Ohio Primary Care in collaboration with National Church Residences to bring you a new standard of senior health care right here in Columbus. With a combined 83 years of primary care and care coordination experience, our partners know what it means to provide the comprehensive care you deserve. You’ll discover what makes us more than a doctor’s office from your first visit to the SeniorSelect Center.

The Comprehensive Care You’ve Been Waiting For

As a non-residential facility, our members see the SeniorSelect Center as a second home. And even though you won’t be living in our center, the dedication and communication that you’ll experience will remind you that we’re always there for you. When you become a member with SeniorSelect, our Life Care 360 program will help to ensure that your transition to our care is as seamless as possible. After five weeks of establishing visits and consistent contact, we’ll work with you to establish a long-term Life Care Monitoring plan.

Life Care 360:

• Comprehensive Life Care assessment, completed with all new members and annually thereafter.
• Life Care follow-up meeting, completed within the first three days of the first visit to establish a care plan.
• Consistent weekly contact for weeks three, four, and five ensures comprehensive coordination.

Life Care Monitoring:

• Individualized Engagement Care Plan for all members.

A Community You Can Count On

At the SeniorSelect Center, our Life Enrichment program encourages a thriving senior community. Every day of the week, we offer activities like chair yoga meditation, cooking classes, Zumba dancing, painting, and more. And, our Welcome Café is always open for a chat with friends.

When we built the SeniorSelect Center facility, we kept two priorities at the top of our list: convenience and senior accessibility. With a fully equipped lab, life enrichment services, a thriving senior community, and specialty services providers all on-site, members can meet all their care needs right here at our Columbus area site. In addition, SeniorSelect members have access to the COPC SameDay Centers. Emergency room visits can be time consuming and costly, especially for seniors with complex medical care needs. The SameDay Centers are available to provide the care you need, right when you need it.
