Mary Gran Nursing Center provides a complete continuum of care including short term rehabilitation, long term care, assisted living, hospice, respite services, and out-patient therapy. The facility is conveniently located a few miles from Sampson Regional Medical Center and has provided rehabilitation services to Clinton and the surrounding areas for over 30 years. Our professional and compassionate staff offers planned care that is patient focused. Patients can access our state of the art rehabilitation services including physical, occupational and speech therapy.
Mary Gran provides a dedicated short term rehabilitation unit with private rooms, televisions and telephones. For those who are thinking about elective surgeries, Mary Gran offers patients a pre-registration program. Patients may pre-schedule their rehabilitation stay following their surgery which decreases any anxiety regarding which short term program they will recover at. The facility offers complex care to patients including those requiring IV therapy, wound care and dialysis services.
Enhanced therapy services a Mary Gran includes diathermy, that assists with decreasing pain, e-stimulation to support improved joint movement and muscle strength and vital stimulation that promotes better swallowing. When a patient is engaged in these enhanced services, they have a more positive experience during their regular therapy sessions.
Planning a safe discharge is paramount to every patient’s successful transition back to their prior living setting.
The staff at Mary Gran Nursing Center are here to make the transition from the hospital to post- acute care easy. Patients who are admitted for short term rehabilitation have experienced an injury or illness that is expected to improve with the intervention of therapy and nursing supervision and return to their prior living setting.
Each member of our interdisciplinary team does an evaluation of the patient at admission that looks at their prior level of functioning, their medical history, their current level of functioning and plans are for a safe discharge. Our short term patients are evaluated for the amount of physical and occupational therapy they are able to tolerate. Speech Therapy also evaluates a patient if indicated.
Nursing provides medical monitoring of the patient’s medical condition, administers medications. Our wound care nurses focus on dressing changes to a patients’ post-surgical site and any skin concerns. Certified nursing assistants support our registered nurses with monitoring of vital signs and medical conditions and provide help to patients with bathing, dressing, eating and transferring. Together, our team supports the patient to meet their goal of being as independent, to celebrate success every step of the way.
When families look to Mary gran Nursing Center for long term care services, they are choosing an extension of their family to care for their loved one. Those who have a chronic illness or disability that prevents them from doing basic living skills and activities of daily living and will need support for their health and medical needs for a long period of time may be candidates for long term care. Some patients are experiencing memory loss, confusion or disorientation.
Our assisted living unit is designed to provide progressive access to care levels that support the needs of patients while maintaining their independence. Assisted living offers a resident the security of a facility with staff support with one monthly payment. Patients may enter assisted living from the community or following a short term rehabilitation stay.
Some patients find it easier to make the transition from short term rehabilitation to assisted living in the same location. This allows a patient to age in place in familiar surroundings with staff they have gotten to know. Because our assisted living beds are located within a skilled nursing facility, residents and their family have a level of comfort knowing that emergency care is close by. Assisted living residents have access to receive physical, occupational and speech therapy with a physician’s order.
Patients have the option to receive physical therapy, occupational therapy and speech therapy on an out-patient basis. Out-patient care eases the transition from post-acute care to home, using the same therapist. The facility also admits patients directly to out-patient services through referral from their personal physician or a physician who provides first report on workers’ compensation cases.

Respite services allow a caregiver the time they need to attend a special event, take care of personal business or take a vacation. The length of a respite admission can be as short as a weekend or can last as long as necessary, depending upon the unique situation of the caregiver.
