McGregor PACE Adult Day Care Services in Cleveland and Northeast Ohio
McGregor PACE provides adult day care services and comprehensive aging support through five adult day health centers serving Cleveland and communities across Cuyahoga, Lorain, and Summit counties. Participants continue living at home while spending scheduled days receiving medical care, therapy, meals, and personal support in a welcoming, active environment. Group activities and time with others bring connection and variety to the day, while transportation and in-home services extend support beyond the centers. Each participant’s care reflects individual needs, preferences, and living situation.
Adult Day Health Services That Support Life at Home
McGregor PACE is a Program of All-Inclusive Care for the Elderly for eligible seniors who require a nursing home level of care but can continue living safely in their homes and communities. Rather than providing residential living, McGregor PACE delivers services through its adult day health centers, in participants’ homes, and through other care included in each person’s plan.
Center attendance is based on individual needs and may range from one to five days per week. Scheduled visits combine healthcare services with meals, creative activities, and opportunities to socialize, giving each day a balance of care and personal engagement.

Personalized PACE Care Coordination
An interdisciplinary care team works with each participant and caregiver to develop a personalized plan addressing medical, rehabilitative, social, and personal care needs. The team considers the participant’s health, home environment, preferences, and daily support needs when determining how services should work together.
The care team may include physicians, nurses, social workers, physical and occupational therapists, recreational therapists, dietitians, and transportation representatives. Participants and caregivers remain involved in care decisions and receive information about changes to the plan. The medical director and interdisciplinary team also review the initial assessment completed during enrollment.
