• Many of our patients return home following a brief rehabilitative stay in our center.
• For those in need of continued, long-term care services, we offer a home-like setting and compassionate caregivers who are committed to meeting the needs of each resident.

Short-Term Rehab
Our comprehensive rehabilitation services help patients get back on their feet and on with their lives as quickly and as safely as possible. Our goal is to help our patients get back home – stronger, more mobile and ready to thrive. Our Comprehensive Rehabilitation Services include:

• Short -term “Rehab-to-Home” Programs
• Physical, Occupational, and Speech/Language Therapy
• Discharge Planning Begins upon Admission
• Licensed Team of Rehabilitation Professionals
• Fully-equipped Rehabilitation Gym
• Outpatient Therapy (may be available)
• Specialty rehabilitation services include Orthopedic, Neurological, Cardiac and Post-Surgical Rehabilitation.

Skilled Nursing Services
Skilled nursing services are provided 24-hours a day by our team of healthcare professionals under the physician leadership of our Medical Director. We provide competent and caring support for patients with complex medical conditions.
Skilled nursing services include, but are not limited to:

• 24-hour Skilled Nursing
• IV Therapy
• Pain Management
• Wound Management
• CHF Management
• Trach Care
• Tube Feeding
• Ostomy Care
• Diabetes Management
• Nutritional Therapy
• Restorative Program
