Bradley Creek Health Center at Carolina Bay, located on the 24-acre campus of Carolina Bay within Wilmington’s Autumn Hall mixed use community, offers a full range of health care services to meet the needs of the individual.  From Inpatient and Outpatient Rehabilitation to Assisted Living, Memory Care, and Skilled Nursing Care, the staff of Bradley Creek provides services for not only the residents of Carolina Bay but also the surrounding Wilmington area.

Our rehabilitation and skilled nursing center has spacious private rooms that include television, wifi, refrigerator, and large private bathrooms with walk in showers. All of which assist in making your stay with us as comfortable as possible.

Rehabilitation
When a patient requires short-term physical rehabilitation, the staff at Bradley Creek Health Center helps them find the strength to overcome setbacks following injury, illness, or accident. At admission, the clinical team starts with the mindset that all patients want to return to their prior living setting following rehab, and works with the patient, their physician and their families to build a foundation of skills that supports a safe discharge. This can include post-surgical rehabilitation, physical and occupational therapy, and wound care as needed.

• The Medicare Certified rehab gym hosts a therapy suite with a full kitchen, bathroom, and laundry room to allow therapists to assist patients in their return to managing every day needs once they return home.
• Modality programs such as diathermy, Tens units, ultrasound for pain management, strengthening, and increasing circulation are also available to aid in your recovery.
• Our car simulator assists therapists to work with patients for return of function to enjoy your return to community events, visiting friends, grocery shopping, etc.
• Bradley Creek offers Pre-registration for rehabilitation following an elective surgery allowing for a more seamless transition post-surgery.

Skilled Nursing Care
Our skilled nursing center provides the highest level of compassionate, professional care for residents. We understand that not every long-term care resident requires constant medical or cognitive management. Many of our residents just need the reassurance of a supportive environment and the availability of nurses and care personnel 24 hours a day, every day of the year.
