Each year, Legacy Beavercreek Skilled Nursing & Rehabilitation continues to invest in our rehabilitation capabilities by developing our professional staff skills, our rehabilitation equipment and technologies, improving upon our patient care and treatment environment, and testing and measuring the effectiveness of our rehabilitation programs. We are proud of the fact that more than 90 percent of the patients admitted to our Legacy Health Services centers for short-term rehabilitation are discharged home and back to their active lives, often in less than 30 days.
MEDBRIDGE …A BRIDGE FROM HOSPITAL TO HOME
Our capability to achieve superior medical and rehabilitation outcomes has been enhanced by the widespread introduction of our MedBridge post-hospital units in a growing number of our skilled nursing and rehabilitation centers. By the end of 2016, 130 of our centers are scheduled to be MedBridge-certified. These distinct units are specifically for shorter-stay patients who still require intensive medical and rehabilitation care after their time in the hospital. These are patients who have experienced issues such as an orthopedic injury or replacement, a stroke or other neurological concern, a heart complication, a chronic disease, a pulmonary problem or cancer. MedBridge is the pathway to take them safely and confidently from the hospital to home.
MedBridge units are Medicare-certified. In addition, we accept most managed care and commercial insurance, as well as private payment. Many of our centers also accept Medicaid.

My favorite thing about rehab here is the upbeat, can do, attitude. When the hospital released me after having my stroke, they sent me somewhere else for rehab. I felt like that place gave up on me. They told me I would never walk again and I believed them. But then I came here and it was rough at first, but therapy and all of the staff here have been amazing. They do a good job. I’ve come a long way. If I were to give advice to someone looking for a SNF, I would tell them to understand that it’s going to be hard, but don’t give up, you can do it.
