Nursing and Wound Care

The Best Choice Home Care Adult Nursing professionals provide nursing care services at home for adults and seniors dealing with chronic illness, injury or disability. These services are provided by registered nurses and licensed practical nurses and are available 24 hours a day, 7 days a week. Our board-certified Wound Care professionals provide technically advanced, surgical and non-surgical outpatient care to heal wounds that resist conventional therapy. These include:

  • Neuropathic ulcers in patients with diabetes.
  • Pressure ulcers due to immobility.
  • Venous stasis leg ulcers associated with poor circulation.

Our Wound Care Certified nurses assess each patient in a holistic manner to determine all necessary interventions. Improved clinical outcomes are associated with companies that have an interdisciplinary wound team. Our team includes: Patient, Nursing, Physical Therapy, Occupational Therapy, Speech Therapy and a Wound Care Specialist. We work in tandem with doctors and nurse practitioners to identify the best treatment plan. Our success is driven by being patient focused.

Physical and Occupational Therapy

Many therapies can be provided at home to speed recovery from illness, injury or surgery, including physical therapy, occupational therapy and speech therapy. Home based therapy allows clients to develop independence with daily activities in a convenient and comfortable setting, allowing families to be closely involved in the rehabilitation process. Our Physical Therapists help individuals regain physical motion and strength after events such as:

  • A loss of balance
  • A fall
  • Difficulty walking
  • Joint pain
  • Back pain
  • Hip or knee replacement surgery
  • A stroke
  • Heart failure
  • A heart attach
  • Any noticeable decline in function

Our Occupational Therapists work with patients and their families to modify, return to or help maintain essential actives of daily living tasks. Examples of these include dressing, bathing, eating and meal preparation. Our therapist will visit the home to evaluate strength, coordination, range of motion, balance, activity tolerance, mobility and the home environment and establish an individualized treatment plan. Occupational therapy benefits are not limited to a specific individual or diagnosis.  Most commonly, individuals afflicted with the following benefit from occupational therapy services:

  • Cognitive deficits
  • Multiple Sclerosis
  • Heart failure
  • Parkinsons
  • Alzheimers
  • Stroke
  • Arthritis
  • Brain injuries
  • Joint replacements
  • Fractures
  • Visual impairments
  • Loss of limbs
  • Deconditioning
  • Weakness
  • Respiratory ailments

Speech Therapy

Speech and language therapy helps individuals improve breathing, speaking or swallowing functions. Speech therapy focuses on the ability to understand words and the ability to use words to express oneself. Our therapists can help individuals who suffer from:

  • Stroke
  • Difficulty swallowing
  • Difficulty speaking

Therapy plays an important role in helping an individual regain productivity and independence. Therapists are a critical part of the health care team.

Preventative Care

At Best Choice, Preventative Care falls under two main categories – Fall Prevention and Diabetic Education.  We’re proud to offer expertise in both areas, allowing for patient focused, effective preventative care.

Fall prevention is a major focus of all care team members.  Fall prevention assessment is completed upon admission and therapy evaluation. Our skilled practitioners will evaluate the home environment and make suggestions for safety. During the certification period, your nurse will continue to monitor safety in the home. Therapy will work to increase independence, safety, muscle strength and endurance.

Our number one goal is to prevent falls.

Our certified Diabetic Educator will assess each patient in a holistic manner to formulate a patient specific diabetic management plan. We will work with you to develop a drug therapy plan that includes blood pressure and dietary changes, for patients with diabetic nephropathy.

Together we will identify barriers to change and implement a patient specific plan. Best Choice Home Care is located at 5701 Elmwood Avenue, Indianapolis, IIN 46203. You can reach us at 317-632-1500 or toll-free at 888-281-2378.
