Social Services Assessment and Care Management

Central Virginia Alliance for Community Living maintains a skilled professional Social Services Department lead by a Social Services professional and includes five skilled Care Managers, Care Transitions Staff, OPTIONS Counselors; and a skilled Social Services Assistant.

Whether you are planning for your own care or caring for aging parents, we can provide you with the unbiased answers and resources you need. Care management provides individuals the opportunity to make their own choices regarding long-term care supports and services through the help of a Care Manager. The Care Manager can help coordinate community services that benefit the individual and their unique situation, and allow them to remain safely in their home.

Care Management process:

  • Comprehensive assessment of the individual’s needs and wishes.
  • Offer information about services available in the community.
  • Support the individual in choosing what services they can benefit from.
  • Develop a personalized care plan which may include services such as home delivered meals, personal care or home repair. While many services are delivered to the home, many valuable supports exist in the community, such as adult day care, congregate meals or transportation to the doctor. Any part of the proposed plan can be accepted or refused.
  • Work with the individual to arrange for the chosen services and supports.
  • Continue to be available for ongoing assistance, coordination, and assessment.
