Creekside Care Center is a 112-bed rehabilitation and skilled nursing facility located in Northeast Columbus, and serves the residents of Gahanna, New Albany, Westerville, and Columbus. Our conveniently and centrally located campus is now owned and operated by Cardinal Care Management, an Ohio based leader in the rehabilitation and long-term care provider industry.
