Village Home Health and Hospice is a joint venture between Maple Knoll Communities, Inc. and Cedar Village. Both non-profit organizations have a long history of serving older adults dating back to 1848 for Maple Knoll Communities and 1883 for Cedar Village.
Village Home Health and Hospice Care was established to further both organizations’ missions. Dedicated and talented staff provide skilled home health services to enable individuals to recuperate safely in the surroundings where they feel most comfortable ….their own home.
Because we offer home health and hospice care together, our clinicians are trained to care for you or your loved one in the correct setting. If your loved one is not ready for hospice, he or she may benefit from home health services. If you’re not completely sure what kind of care is needed, we have the expertise to guide you toward the best answer.
Our hospice care is specially designed to comfort patients and family members when patients have reached a point in their lives where the priority of care turns to ensuring the patient is at ease, rather than curing the illness. Under the direction of a physician, our interdisciplinary team of clinicians offers a special kind of caring that encompasses the medical, social, emotional and spiritual aspects of treatment.
Your personal Physician and our team individualize a plan of care to meet your special needs. Services for Home Health and Hospice may include but are not limited to:
Our professional staff is available 24 hours a day, 7 days a week to accept referrals and answer your questions about care.
Payment Options:
Medicare, Medicaid, managed plans and most commercial insurances cover skilled home care. We will verify your insurance before our services begin so you will know upfront of your insurance’s coverage.
