Alliance Home Health Care was established in 1987 as a home health care company. For over 17 years, we have dedicated ourselves to helping the older adult live life to the fullest … independently. Alliance Home Health Care furnished in-home personal and medical care as long as needed, from short visits to 24 hours a day.

In October of 1996 Alliance opened the Alliance Adult Day Center, which provides an alternative health care option for dependent adults. The Alliance Adult Day Services provides quality daytime programs in a caring, loving, safe environment. In 2004, we expanded the Adult Day Services to the Westside of Indianapolis and to Munster, IN. Through coordinated care and support for clients and their families, Alliance enables individuals to remain in the safety and comfort of their own home environments for as long as possible.

Alliance also serves clients transitioning back to home from an assisted living setting or from a hospital, nursing home or rehabilitation facility.

Home Health Care

We offer a full continuum of home health care services including total coordination of medical and personal home care needs. Our goal is to provide our clients the freedom of remaining at home and provide their loved ones peace of mind. Whether your loved one needs nursing care or occupational, speech or physical therapy, Alliance employs the most qualified professionals who are dedicated to treating our clients just like family. Our home health care services include:

Personal Home Care

Our non-medical, personal home care aide, attendants and companions visit our clients in their homes as often as is necessary; for short visits or 24 hours a day as a private duty companion. They provide emotional support, caregiver assistance, light housekeeping and companionship to help senior and elderly adults function independently in their own home environments.

Geriatric Care Management

Alliance Care Advisors helps seniors and their families manage through the complexities of aging. We can help with everything from getting information organized to oversight and coordination of care in a crisis to ensure that your clients and their families’ needs and wishers are met. Our plans are personalized, ad our services are tailored to each family that we serve. We solve problems and address the issues of most concern to you and your family, helping you understand your options and make the right decisions. Alliance Care Advisors services include:

  • Assist with implementation of physician recommendations
  • Help navigate complex treatment plans
  • Provide crisis management during hospitalizations and other emergency situations.
  • Protecting your resources by reducing hospitalizations and avoiding unnecessary services
  • Addressing difficult issues like safety concerns managing multiple diseases and cognitive decline
  • Offering a secure web-based portal for seniors and designated family members to view current health care records and health status
  • Plan health care provider visits to make sure key concerns are discussed
  • Attend appointments to document physician recommendations
  • Advise on disease management and wellness strategies in accordance with health care provider recommendations
  • Explain how to integrate physician recommendations and treatment plans into0 daily routines to improve overall well-being and quality of life

Dementia & Alzheimer’s Care

Alliance Home Health Care developed the Nostalgia Program to treat Dementia by engaging our clients in recreation, promoting relaxation, encouraging reminiscence, building relationships, and providing resources to our clients’ families. We customize programs based on clients’ past or present interests. We also offer client-centered care management to link and coordinate quality services, referrals, and consultation.
